Shooting Forum - Firearms licencing information

Hi all, I'm sure I have come across this before but can't find it now - can anyone tell me if there is a specific form for getting a second licence on a firearm already owned and licenced by another person or is it the standard FCA1 with a cover letter?

FCA1 is used and in section 4.1 you insert the cert number of the person currently licensed. Same with section 3.3. I fill in this when I'm getting a joint license. Use the private source boxes and supply the person's name, address, contact info and cert number.
The rest is as normal.
