I want to get my blood vitamin/mineral levels, kidney & liver function etc checked out, basically a general checkup which would highlight any more obvious health issues or nutritional deficiencies. Is this a standard procedure and what should I ask for? Is this something any GP can do?

Yep. My GP did them for me a few months ago. Covered all of that and more.

Is this something you'd need to ask a GP for in advance or can they do it pretty much on the spot (i.e. regular appointment) and do you need to fast?

They'll usually ask you to fast if you want to be tested for diabetes or cholesterol. Iron as well though I'm not entirely sure if it's as important for that. Better to ask the GP in advance though. 8hrs would be enough of a fast so if it's first thing in the morning you're being tested, it's easily managed
