Illness Benefit..First Timer Query…

First time ever claiming and I’m a bit confused…I was only asked what date I was claiming from? My sick cert is for 3 weeks yet it never asked what date I am due to return? Also didn’t ask me for evidence of my sick cert? Am I missing something?! Did I may be fill it out incorrectly? Thanks…

Your GP needs to have sent, electronically or otherwise, a certificate on your behalf. Thus there’s no need for you to mention that. You won’t get IB if he/she hasn’t sent it.
The GP has written on that cert how many days/weeks you will be unfit for.
