    First time ever claiming and I’m a bit confused…I was only asked what date I was claiming from? My sick cert is for 3 weeks yet it never asked what date I am due to return?

    Also didn’t ask me for evidence of my sick cert?

    Am I missing something?! Did I may be fill it out incorrectly?
