Appearing before the Tribunal last year a local social worker described how she had created the report about the alleged incident of sexual abuse involving McCabe and amalgamated two separate and unrelated incidents into a single report. Gardaí, the tribunal was told, hadn’t responded to queries as to whether the allegations were the ones that had been investigated years earlier.

The social worker said she only realised two weeks before the tribunal began that she had made a “catastrophic” mistake by making the files – no-one had contacted her after Tusla was alerted to the error in May 2014.

I wonder is this social worker still in a job? If this is true, this is indeed a catastrophic error and gross incompetence. Frightening to think anyone's life could be destroyed in this manner.

What did the social worker do exactly?

The FACTS we do know already paint a bleak picture of her not wanting to rock the boat and having zero interest in investigating the real allegations against poor practices. That says enough.

Can you give me a source for this fact? Incidentally, have you read the tribunal report(s)?

Some sort of "cut and paste" error from two other unrelated cases were put into McCabe's file.

    I'm sure that happens everyday

I would suspect the same. However many posters here think TUSLA deliberately did this to the McCabe file...because...because...

She was pretty much fully vindicated by a tribunal of inquiry - what are your reasons against the appointment?

    Try to be specific.

Tribunals of inquiry can be wrong you know, something to think about. Just because some piece of paper says she did no wrong, doesn't actually make it so.

Not sure what the big deal here is - isn't a promotion what happens every senior political appointee when they fail?

Perhaps Phil Hogan or Pee Flynn could throw light on this?
Jesus never was a nickname so apt.

I've read into this a bit since I last posted. It now looks to me that the original woman accusing McCabe (Ms D) of which he was cleared by the DPP, got counselling from Laura Brophy, who then cut and pasted another more serious accusation from a different woman (Ms Y) to a different man into his file. So it does look like a genuine mistake. A horrendous mistake. How many more mistakes like this have been made?

    https://www.thejournal.ie/disclosures-tribunal-timeline-4081567-Jun2018/

The whole Ms D case seems to be either
    a. McCabe genuinely abused her in some way and got away with it or
    b. Ms D suffers from severe mental illness and paranoia centred around McCabe.

She seems particularly determined in her convictions. Interesting is she strenuously denied ever making the claims listed in the file. If she was just out to get McCabe, why would she be so honest? What would be really interesting is if Ms D came forward and told her story.

16 May: The Tusla notification is sent to office of Nóirín O’Sullivan. Her secretary Walsh said that he recalls bringing it to her attention. She told Tribunal: “I can’t recall that… I have no recollection of actually seeing or reading the content of this file.”

So Nóirín's secretary shows her the Tusla report alleging a rape my Maurice McCabe and she can't recall seeing or reading it.

Ya she definitely not lying there.

She's got a prestigious UN job. I'd imagine it's not given to people with a history of wrongdoing.
    Fair play to her.

10 June: Taylor is removed from his position as head of the garda press office, and is reassigned to the traffic corps. It’s his assertion the smear campaign stopped once Callinan resigned but that O’Sullivan had been aware of it all along.

2015 summary: The main headline here is the O’Higgins Commission. At a time when Nn O’Sullivan was looking to support Maurice McCabe, she also gives the go-ahead to challenge his motivation for making his claims at the commission.

Whether she was a part of the plan to smear McCabe or not, she did nothing to help him only hinder him.

    She should be ashamed.

She was very patriotic.....wanted to see reforms in place....but suddenly when she was in the job long enough to qualify for the "big bag of money" that goes with retiring from the job.... in the public interest a new face would be appropriate.......bye bye ....€€€€€€€€€€ here we come
