There are ones that you can use for just the cock, then ones that are for the cock and balls. I wonder was this one designed just for cock but he decided to stick the balls through as well, and that's when the trouble started.

I dunno, I mean I always thought cock rings were made of rubber, at least any of the ones I've ever encountered have been. At least with a rubber one, if it looked like it wasn't going to come off normally, you could easily cut it. Probably wouldn't even need to go to the hospital (and avoid the risk of the story and pics of your sexual misadventure making it onto the national news)
