are you shooting a 24gram or 28 gram shell ,your gun in not cocking the second barrel due to lack of recoil .

try slapping the butt plate to cock the second barrel after the first shot .

there might be just dirt in your action ,take off your stock to have a look.

Hey pkaner. I have limited and i mean limited knowledge of firearm repairs. The Beretta trigger mechanism works of a weighted and inertia based system. When you fire the first barrel the weight is shifted from the inertia of the recoil and triggers the second barrel. You should try loading the gun and firing the second barrel first. You already know it will fire the first and not the second so try it in reverse. If it doesn't fire try it again, using the second barrel first and then manually switch to the first barrel and make sure it fires. If it works on both barrels by manual selection then it would seem as though there is a problem with the weight not triggering the second barrel from the recoil. This is out of my league of technical know how. I took my 686e apart one day to clean around the mechanism (i do it every few months) but i would leave any repairs to a professional as there are too many springs, rods coils etc and i would be afraid of not putting it back together properly.

Best advice try out what i suggested then if that gos as i described seek a professional and tell him/her and they should be able to sort it out.
