Medical card renewal form - HELP!

Hi everyone,

In July, I changed address and moved from Kildare to Dublin. Last week, I realised that my medical card expires at the end of this month. I rang the HSE who informed me that they had issued a renewal letter to my old address in December but obviously, I didn't receive it as I had forgotten to renew my address. So they sent me out a form to my new address. I expected it to be a renewal form whereby I wouldn't have to input all my details again and send in all my documents yet again but it is a fresh, application-type form like I filled in when seeking the card first years ago.

Is this the norm? Does everyone renewing have to start fresh and send all documentation in again? I also did not receive any letter containing a password to see the progress online?

You will have to complete the whole process again same thing happened to me,my wife was taking into hospital and I forgot completely to print and send off the renewal within the time limit end result was start all over again.

What is the time limit? My card isn't up yet but they didn't send me a letter with the online application password.

When you didn't return the form they sent in December then they presumed that you no longer required a Medical Card
If you failed to notify them of your change in circumstances 8 months ago they will require documentary evidence to support your current outgoings and income.

The sooner you return the form the quicker youll hear back.
