Cost Plus Sofas Insurance

Can anyone help me on this?
My mother bought a sofa deal from Cost Plus a couple of years ago and she also bought a 5 year insurance deal with it. We can't find the policy we had anywhere at home but there is some damage to the suite now.

Does anyone know the insurance company name and also what's the best procedure to claim from it without having all the details as i know insurance companies try to fob you off at every chance, not having the policy at hand would make this easy for them.

It might be from Guardsman (http://www.guardsman.co.uk/en/index.html) or Furniture Clinic (http://www.furnitureclinic.com/)they are regulated by the Financial Conduct Authority in the UK so if they try to fob her off they can't even if the retailer in this case CPS went into administration. they have to honour the cover as per contract
