Tilt is lifting the outboard up for when you put it on the trailer or if you come into shallow water.
Trim is adjusting the angle of the outboard leg when your out on the water to bring the bow of the boat up or down.
Power tilt and trim is mostly fond on bigger engines as they can be very heavy to do by hand.

on speedboats and the like it'll add a few mph on to the the WOT too. Had a 70hp once with no tilt and trim and it was a killer to lift it up by hand.

If you are thinking of getting a bigger engine with tilt and trim I would recommend doing a powerboat course they will teach you all you need to know and it will pay you back when you go to insure your boat. http://www.powerboatschool.ie/level_1_2_national_powerboat_certificate.php

does each outboard need a certain one or is there a universal fitment?

They are different to each outboard some with one ram some with two, you can get a jack plate that will fit most outboards.
