Self-employed with PRSI'ed wife.

I started self-employment on 01/10/2012 and earned 520 Euro that year. My wife was on PRSI as she works for a company. We were given the Form 12 but where do I put my earnings? Or do I use Form 11e? Thanks a lot, I know it's a bit late :D
