Medical Report Form D501 , what's this?

    I'm about to renew my provisional license , for the 3rd time - not a driver , but I like to have it - I got the renewal forms in the post and it says I need to get a D501 form filled by my doctor , I'm 30 years old , have good eye sight and no medical aliments , do I need to get this form filled out?

    I'm afraid so.
    I was in on Friday to apply for the truck provisional and was refused untill I get that form filled in.
    Basically €45 for the doctor to sign his name.

    Was it your first provisional? This is my 3rd , I definitely had to get a opticians report when I got the first provisional - Did you tick yes to any of the questions asking you had you a medical condition? I have been checking this out , and I appears that the D501 only required if you tick "yes" to any of the 'Health and Fitness' questions... I could be wrong.

    Ticked No.
    Not that it mattered, the woman behind the counter just looked at the from I handed her long enough to see what it was and asked for the medical report.
    She didn't look at the back of it where the actual form is.

    For the Truck yea.
    I needed a report before but I can't remember was that 18 months ago for my full car license or 2.5 years ago for my provisional.

    Its the C truck.
    I know I needed one for the car too but I can't remember if it was for the provisional or full.
