Help needed to get order form onto website.

    Hi, i'm currently building a website for custom sticker printing. I need a form which will allow customers to do the following.

    1. Enter the dimensions of their sticker
    2.Enter the amount of stickers required
    3.Calculate the price of the order.
    4.Upload an image with their order
    5.Pay via paypal.

    I know its a long shot, but does anyone know how I could put this onto my website. My current order form is here but its missing the paypal, and calculation part.

    Any help will be very much so appreciated.
