Lost theory test cert - what to do?

    I passed my test around a year ago.

    Since then I moved house and had to deal with some personal stuff.

    anyway I can't find my theory test cert.
    Is it just a matter of popping in and getting a new one? Like will they have my record on the computer?

    Also, what else do i need for my provisional? eye test? Anything else?

    What if I lose my pass certificate?
    You can request a duplicate certificate by calling 1890 606 106 (Lo Call Rate) and the Customer Service Representative will ask you for you personal and exam details such as your name, address, date of birth, and test confirmation number (provided in the Confirmation Letter / Email you received for your test).  Payment of  €15.00 is required and can be paid using Laser Card, Visa Card or Master Card.
