How do I register for a VAT number and also how do I register a new company name?

For VAT Registration you need to complete a TR1 form which you can get from any Revenue office.

It is advisable to think about registering before doing so because unless your annual turnover exceeds €37k per annum you're under no obligation to register for VAT.
Also when you register for VAT, you will have to make VAT Returns to Revenue bi-monthly.
So think about whether you need to register for VAT.

In respect of registering a limited company name, I would recommend thinking about asking a company formation specialist firm. They can compile all the paperwork and register your company with the Companies Registration Office for
about €250.00.
