Registering as Sole Trader - TR1 form

I'm doing some contract work so I have to register as a sole trader & I have a couple of questions about the TR1 form if anyone could shed any light on them for me? (on hold for Revenue for a long time this morning, going to try them again after lunch).

- In section B, I'm supposed to tick the box to register for Income Tax amn't I? (I know this is a seriously dumb question but I'm a bit of a tax n00b). Then as my income is from provision of a service I tick the box for 'Trade'?

-The form asks 'To what date will accounts be made up to', should I just put the end of 2011?
