    Call them and find out.

Yep you can, as long as you have all the forms filled out correctly, there should be no problem.

    Yes it is.
I had to get someone to collect mine 4 months ago.
The man in the passport office told me to write a letter authorising my nephew to collect it, and to give the letter to my nephew when he was collecting the passport. I had to specifically name him as my collector in the letter, and he had to bring ID (passport or drivers licence) as proof. I also gave my name, address & mobile no in the letter, just in case anyone from the passport office needed to call me to verify. If you are writing this letter yourself, on behalf of your girlfriend, make sure that the signature is similar to the one in the passport application form.

You should bring the receipt as well if you have it.

    Bump:

So is it possible for me to go to the passport office and hand in forms on my partners behalf and collect their new passport?

Should I bring a letter of authorisation as someone posted above?

Yes. All further queries go to the Passport sticky thread.
