    I know there is a specific thread on HEAR already but I need a specific answer to a specific question.

    What exactly are the forms needed?

    I know that you need a form from the following:
    - Department of Social and Family affairs
    - Health Service Executive

    You also need a statement of your parents income, what form is this? (P21, P60 or something else?!)

    Is there anything else needed?

    nope not less you have an accountant? and its a p20 form needed

    If your Parents:
    Are Self-Employed: All you need is the Notice of Assesment
    Are Employed Normally: All you need is the P21

    Along with the DSFA and HSE forms ya mentioned
