My home machine is taking an absolute age to start up and even after starting, cpu usage is pegged at 100%, but as far as I can make out, the total cpu usage of all running apps is around 10%. Last time this happened, it was a nasty spyware attack but:

    1. There was more evidence of this attack last time; pholmes.exe, random popups, etc
    2. I've installed absolutely nothing on this machine since I cleaned it up.
    3. I've got SpywareGuard running

I've left it running an ad-aware scan in safe mode before leaving it this morning, but I just wonder is 100% cpu usage with no evidence of such a pretty sure sign that I've been infected with something?

I take it you have checked the taskmanager to see which processes are using up the resources?
