WTF is it? A lot of sites refer to them, but none actually explain them. Is it just a campaign done in co-op mode?

You score points for each kill you make in the campaign and using the skulls can increase the value of each kill, but also make the game a lot more difficult. You can do it either as co-op where you are aiming for a team score, although that does not then count towards the achievements for the metagames, or as a "competitive" match where the one with the highest score wins.
