I'd suggest Joe Keaney and the Cork Hypnotherapy Clinic. My sister studied under him for her diploma and she highly recommends him.

Not Cork but Rhoda Draper at above seems well versed in hypnosis. Not sure about TFT Thought Field Therapy maybe worth a try

You might also consider going to an NLP practitioner. NLP seems to be very effective with phobias.
