One suggestion but they're pretty rare is the Volvo V60 Cross, it's an estate but with slightly higher ground clearance and comes as standard with all wheel drive.

What's the budget?

Only options coming up for me in estate hybrid are all high end stuff. Cheapest being the Volvo V60 PHEV T6 (350hp) Plus Dark AWD @ €67,045 before delivery or options.

Good news is that you can probably get one from stock as Volvo bring them in, but very few people but them - there were 10 around in April, but I know at least one was registered as a 252 demo so it's possible other dealers did the same.

Actually what do you even need 4WD for?

Can you get a primary medical cert and avail of VRT/VAT reductions I wonder?

I’d be looking at a Tesla Model Y Long Range Dual Motor.

€52990 incl SEAI grant, but there’s €3500-4000 VRT to claim back and around €9000 VAT, so I reckon that would leave it around €40k

I know you say you’ve no interest in an electric car, but these have great range, there’s a 4 year warranty, they drive better than any diesel, and they have 4WD.

They don’t sell them in Ireland and are only available to emergency services in the UK

Well, that explains a lot. I was keen on one of these years ago, I never worked out why they were so rare. I'm pretty sure I saw one on Irish plates in Clare a few weeks ago if that's possible.
