No log book after buying car

Mate bought a used car, signed the book etc but previous owner is saying he sent book but it hasn't arrived in Clare. He will not play ball anymore. I know the process re going about claiming ownership - have it verified by dealer, peace commissioner to sign declaration etc. Anyone know where I can get the form online?

Thanks. I had seen that one but it looked almost from the last century.
Is that definitely current.
