Pet Cremations Ireland has that exact item.
They have a store with lots of keepsake items.

They call them tribute boxes. I got Lucy’s ashes back in one with her name engraved on it, I had planned to scatter some of her ashes but they came back in a little paper parcel/bundle inside the box and I couldn’t bring myself to untie it! the one we got had a small space for keepsakes and I have the card we got from the vets with her paw prints and a small bag of fur inside.
