In a lot of cases it's psychological I think. I'm hesitant to even tell this story, and if it's deemed inappropriate then flag it and get it removed, but it's relevant to the discussion.

The man who wrote Fight Club has a true anecdote about how he was doing a reading of a short story somewhere and a lady came up to him afterwards and revealed to him her deepest and darkest secret. From personal experience it is common to become distracted or self conscious during thrusts due to invasive sensory elements. If the male becomes self-conscious during rutting the whole endeavor has been put at risk. If the female recognizes the male’s lack of confidence she too becomes self-conscious, and will start to search the male body for undesirable elements (essentially to lay the blame). While the clitoris is generally more reliable, every woman I have ever been with in around 30 years has been different and what works with one girl will not necessarily work with another.
