    You have to play 60 minutes I'd imagine. Just because nobody came on for you doesn't mean you haven't gone off.

    Edit: OK going by what others are saying maybe I'm wrong.

    He can't be subbed now. If he goes off for treatment he can return at anytime so you can't count him as subbed off.

    You could just go off for treatment
    Unless you tell the ref you are not coming back on you are free to do so at any stage whether you do or not
