Ive been told game begins with white "in hand" which means 21 balls on table but others say game begins with 22 balls on table (after you've placed white in D), which is it.

"in hand' doesn't mean literally in your hand, it means you can put it anywhere in the semi circle. Since the game can't begin without the white on the table then the answer is 22.
