Medical card application

hello, I am applying for a medical card. I have an Irish Life 5 year investment plan and a private Zurich pension. At present I am not receiving any income from these . do I need to mention these on the application form?

It's not just income. They do state on the form that you should list any savings or investments which presumably includes your Irish Life plan. Pension funds may be exempt, but if in doubt I'd either ring and ask, or else list it but make clear what it is.
