Online forms

    I work for a large company that still uses paper application forms for clients. I'd really like to move towards an online form that can download in csv format or something to a spreadsheet. At the moment, there's a ridulous amount of work manually filling in spreadsheets. I have a few requirements though:

    1. The form has to be reliable and secure.
    2. It needs the ability to save partially completed information to allow a client return to it on more than one occasion if necessary.
    3. It needs to be somewhat customisable to take on the company's logos and general aesthetics.

    Has anyone any experience here? Not my area at all so I'm stumped.
