"Notice of Assessment" - what does it look like?

Hi, I've been asked to provide one of P60/EDS/Notice of Assessment for several years. As I've been self employed I don't think I have either of the first two. (under Myaccount in ROS, next to EDS it says "not available.") So I wonder if anyone could tell me what a Notice of Assessment looks like? I can't find anything sent…

So I wonder if anyone could tell me what a Notice of Assessment looks like? I can't find anything sent to me from revenue with that exact title, nor anything under Myaccount. I do however have "Acknowledgement of Income Tax Return and Self Assessment" and also another document called "Self Assessment - Chapter 4 of Part ...". Is it either of those? Thanks.

Yes your self assessment chapter 4 would be the notice of assessment
