Refund of emergency Tax Form.

I was employed in a temp job for a few days contract 5 months ago. I was paid for the work but was charged emergency tax on my wages. It was not refunded to me. Is there a form I need to complete and submit for an emergency tax refund? Who is the form ( if applicable) submitted to? Thank you
