Tax Refund From Laser Eye Surgery

Hi,

I recently had laser eye surgery and paid a total of €5,125. Supposedly 20% is refundable as I am not covered by private health insurance but I have no idea how to actually claim it back.
I've looked on revenue.ie and even using mygovid but I still haven't found anywhere to make a claim for medical expenses.

Has anyone here done it or can you tell me what I need to do?

I had read that but found it confusing because I thought revenue were doing away with P45/P60's?

If you file Form 12 as an employee then you can do all that through MyAccount @ Revenue.ie
If you file Form 11 then it goes on there via ROS.
The separate Med1 form has gone. Medical Exps are claimed on the relevant Tax Return for the year concerned.

Clarification - if it is in 2019 you need to do it on the 2019 return after the end of the year. If it is in 2018 you can do it now.
