Fair deal application

    Six years ago my cousin successfully applied for the fair deal for a relation. Now that person has died and he has been told that on the application he failed to declare under 'cash assets' a sum of 10 or 11k and they (HSE) want a percentage of this. He can't recall what exactly he put on the form 6 years ago but believes that if it was left blank or zero that the form stated something along the lines of 'if cash assets do not exceed 30k then skip to section...'
    The new form does not have this but does anyone know if the application form has changed format in the last 6 years?

    Ask the HSE for a copy of the form that was filled in 6 years ago.

    He doesn't want to contact them unless he has to and thinks that once the information was entered onto their system it's unlikely they would hold onto the original for 6 years. He really just wants to know if the form has changed in recent years.

    Perhaps your local citizens information office may have an old copy?
