disable voice mail in nokia lumia 520?

    I thought i had posted this but do not see it. Apologies if it is posted twice

    How can i turn of voice mail in nokia lumia 520? Phone icon/settings/call forwarding is off. But still the call goes to voicemail if not answered.

    Sometime a call goes to my mailbox without ringing when i am not using the phone and does not leave the number as a call missed, or even call missed without number if the number is private

    Any help would be appreciated
