    Get a new physio ...

What has the physio said? If the physio is not giving you a proper diagnosis or rehab / prehab exercises to follow in your own spare time and getting you to come back for more 'treatment', get a new physio. Seriously.

We can't really diagnose here, id question if a 'knot' in the shoulder would cause 4 physio visits.

If it is a case of muscle tightness, do you own a lacrosse ball? have you tried self myofascial release at home?

If not buy one (any small hard ball will do), I find it very good for releasing knots in the upper back.

I had ongoing tightness in the area of my right shoulder blade for months,I got a lacrosse ball and started doing some massage with the ball against a wall or on the ground,I just find the tight spot and applied pressure for 60 - 90 seconds. Hurt like hell first few times but within a week or two i was pain free and much more mobile. One of the best fitness tools I've ever bought and only cost me a 5er.
