    Educating / empowering women and providing better health care work even better.

    It's less likely there will be "spare" children in case some of their siblings die.

    One follows the other with urbanisation but you are correct if women in more rural areas of the developing world had better access to healthcare and education doubtless it would have similar outcomes. Although the anonimity you get in urban areas quickly diminishes patriarchal powers esp in periods of rapid growth, this sort of control will remain in rural areas for quite a while longer. Capt'n Midnight can't be subject to the same influence if very few people know who he/she is and if the behaviour that people want to influence has become commonplace.

    Re comparing populations at different times.
    Ireland used to have 8.4 million back in 1841
    DRC lost half it's population under the Belgians.
    Large parts of Tanzania and Namiba were depopulated under the Germans.
