Can you help me,when it says for medical card No. do I put the PPS number or the GMS number? And when it says valid to :do I write 08 2013 or do I write Aug 2013? Aug=August ,the 8th month Thanks !

Medical Card number is the number on your medical card or GSM number. I am sure they will figure out the date in either format.
