Have an old netgear TA612V which I want to plug into the UPC cisco router/modem to use for making voip. I need to put in a user/pws into the TA612V, am I right in saying there is no username in the UPC box

Yes, the username / password / SIP server information will come from your VoIP supplier (e.g. Blueface) - the cable modem does not need any configuration. UPC's phone service is not the same as SIP based VoIP, and can only be configured by UPC themselves anyway.
