Returning Irish reg car to UK

Bought a car in the UK a few years back. Car is now Irish regd. Does anyone know what forms I need to complete to return the car to UK. I have the usual rf100 but am not sure what form to complete to get the car out of my name. Can't get through to Shannon.:confused:
