PT Screening and Waiver Forms

    I just recently qualified asa PT and was wondering about the best way of designing a scrrening form and waiver as I would like to set up on my own.

    Is there any other form I would need and would these cover me if I was to train a football team.

    Maybe Transform or one of the other trainers would be able to shed some light on this.

    A quick google of 'PARQ' forms and you'll get some standardised wording to base it off, you can add any specifics in yourself you feel you need covered.

    Thanks cmyk.Just wondering does anyone have a good template that they use that i could work off.

    Is it just a par q and waiver thats mainly used?

    may be worth checking in with your insurance policy provider to see if they have any special requirements for your cover?

    Who do I go to for insurance and will insurance cover PT aswell as teams.

    Also does a solicitor need to look over he waiver and screening form it so it would be legally binding.

    Is there any solicitors that deal with this

    A waiver is not worth the paper it's written on. Legally people cannot sign away their right to compensation if you injure them so waivers are something that everyone fills in but it's useless if you find yourself at the wrong end of a law suit.

    A waiver is not worth the paper it's written on. Legally people cannot sign away their right to compensation if you injure them so waivers are something that everyone fills in but it's useless if you find yourself at the wrong end of a law suit.

    That may well be true in certain circumstances, but in the event of something like non-disclosure of an underlying medical condition etc. I'll still be demanding my clients sign one, whether that holds up in a court of law is another discussion but I'd want as much in my corner as possible.
