What's the process for instant email updates for threads where posts may be pre-moderated? e.g., I subscribe to a PI thread, there may be multiple posts by unregged posters after that. Does the system email me when the posts are made? When they're approved? Or does it email me at all? There's been a couple of occasions…

I think you get emailed when the posts are approved - you definitely don't get a notification when a post is held for approval.

Actually, I'm less sure now that you get a notification when a post is approved. That seems to be a situation where a thread notification slips between the cracks.
