I would have regarded Irish Rail as one of the safer railway networks but according to the report a disaster is just a matter of time. Maybe a bit OTT but worth discussing?

Probably a bit ott but if you cant rely on a trained qualified driver to drive and stop safely- then why bother with a driver ... a controller and computer in an office could probably run a good share of the system ...
