Do wall vents need to be ducted after insulation?

A house that was insulated had round holes drilled into the walls were just covered with hit and miss vents and I’m wondering is this adequate? I can see the hole is plastered around it like a tunnel but no signs of internal tubing or ducting or anything like that. Should this have been a requirement from the builder…

Should this have been a requirement from the builder before the vents get placed? Also extractor fans in bathrooms should they also be ducted? Or does simple placing a fan over the hole work? Not sure whether to flag it or leave it.
