Actually, that's one thing I would change: I'd love to see more big studios using digital distribution and DLC models to try new things. I still think The Last of Us: Left Behind was a more successful and idiosyncratic experiment than pretty much anything other AAA studios have managed recently, and think those sort of smaller side projects - whether as DLC or standalone titles - could have wonderful creative dividends.

Probably not what you are talking about, but I wish more games would follow how Half Life did expansion packs. Seeing the same story from a completely different perspective (we see it from 3 sides in total) is a very interesting approach to DLC. I know GTA IV did it too, and I'm sure there are more as well.

    Escort missions.

It's inevitable that the gormless shítehawk you're supposed to protect will either
    A) not walk alongside you.
    B) will wander off and get shot/run over
    C) walk right in front of you as you're about to shoot an enemy thus leading to you shooting him/her and failing the mission.

Most frustrating aspect of modern games for me anyway.

Not to mention the "Walk alongside this character while they spout exposition" parts of missions, which usually end up with either the character not going at a proper speed meaning you have to walk/run/walk/run, or the get stuck in something and you have to try push them out.

I have to say modern escort missions have become a lot better. Games like Enslaved, The Last of Us and Bioshock Infinite have you escorting characters around the whole time without them ever really being a hindrance. Devs are smart enough to know we don't want our unarmed helpers charging into fights, blowing our cover or otherwise getting in the way

Witcher 3 is really good at this as well, there's been a few escort type quests that were really well done, and it kinda makes you wonder if it really is a bad quest type, or have other games just handled it incredibly poorly.
