Convert PDF to physical item

This may sound strange but is there any way i can convert a PDF design that i created on my computer ? the material i want to use is foam. If i printed out the design as paper would i have to do a cutout of it over a piece of foam material?

kinda but i have a PDF of the size and shape of it, it has two circle holes on each side of it and i want it to be the exact shape and size as on the PDF, would i use some sort of hole puncher?
