Getting pregnant on the pill

Ok I took notice of this on another thread. How many women have gotten pregnant while on the pill, be it the combination pill, progesterone only, etc. Out of curiousity, how many women (myself included) were very good at taking their pills but got pregnant? Or did ye get sick/eat bad food or just accidently forget?
