    ..a tattooist to do something like this.
    Not the whole thing but all the red bits

    Not to dash your hopes or anything but do you really think that'd look at all like it does in the context of the picture when taken out? It'd just look like a big red squiggle... I'm sure it's possible to do something like a kind of "red smoke" effect which could look pretty cool..

    Actually now that I think of it a plume of red smoke could be a pretty cool tattoo...

    just go to a decent tattooist and it shouldn't be too hard for them, if that's what you want. Just look through various artists portfolios and look at things like shading and things in the pictures and decide on an artist.

    You'd want to make the horsehead shape clear I'd say, if you want people to recognise it.

    Edit: But I think it could be a cool tattoo if done well.
