What's the Full list of FTV channels?

Not FTA, but FTV. Can someone post a list/link to all channels available via an FTV card, such as the Free2view card? I'm sure it's been done before, but it might be no harm to bring it back to the top again. (And I can't find it anyway!) Thanks, MRed
