disability allowance questionnaire letter

    Hi ,

    I am on disability Allowance since 2017 , basically I suffer from bad depression and anxiety along with a few physical symptoms , since I cared for my mother with dementia for years. It really can be debilitating .

    Anyway , after caring, my mother ended up in a home , I got the DA . though my stress levels were ten times worse the way the made me feel. so my doctor ,said he would send me to a psychologist , but I heard nothing about it and then covid hit. now I think its over a year since I saw my Doctor , there's always a stand in

    now I got a letter in the post , its like a questionnaire about how I cope with walking , simple questions like how does my disability affect me watching TV ! stuff like that !

    Anyway , they also asked about referrals to specialists etc , I was referred but nothing came back . they have a small little box to add answers !

    Should I just write my own letter explaining how I actually feel

    has anyone had this questionnaire , and an end process of what happened ?

    does this mean I'm going to lose DA , my stress levels are through the roof , actually initial DA gave me even more anxiety going through the process .

    These sort of letters are pretty common. Your case is under review. It does not necessarily mean that you lose DA.

    I would answer as truthfully as possible. Describe in detail how your condition affects your daily life. Describe it as detailed as possible and don't be ashamed of disclosing very private details- otherwise SW does not know how crippled you are- and comes to certain other conclusions. A long letter written by yourself are very helpful.

    Never mind the fact that you are on waiting lists without end. SW is aware of the disastrous state of our health service.

    Are you sure that you have to fill out this form entirely by yourself? As far as I remember all those questions in regard to walking, stairs, etc. is filled out by your doctor. Check again- there might be a referral of the whole process to your GP.

    In any case I would discuss the matter with your GP- he might be able to supply you with some more paperwork to back up your case.

    Hi , thanks for your response , The questions are pretty basic , like yes or no , and a small box to comment on , It doesn't saying thing about my doctor filling them out , its two pages of yes no .

    yes I think I can explain things better in a letter .

    Sounds like a random review letter. Means tested payments can be reviewed at any time..fill it in honestly and you'll have nothing to worry about.
