Can an Insurance company approve my claim and then later tell it was a human error and cannot be paid based on their terms and conditions

See the little "E&OE" written in nearly every set of terms and conditions.

    "Errors and Omissions Excluded"

So they found an error. they can correct it.

I bet a zillion dollars if it was the other way round and you found they declined the claim in error, you be making sure they corrected it. 😁

    Works both ways

I would be curious to know a little more detail about the facts of what actually happened.
